Before you take your new companion home, we want to make sure you are able to properly take care of your pet. At Second Chance Pet Rescue we take animal safety and quality of life seriously, and it is our commitment to the animals well being and for them to find their fur-ever homes!

* You MUST visit the shelter during business hours to meet the animal in person in order to adopt.

This application does not place a 'hold' on the pet you are interested in.

Submission of this adoption form does not guarantee that your adoption for the animal is final. It only allows us to pull up the form in office for a first come first serve basis, and for our records.

By filing it, you understand that you are only consenting to giving us a record. Please visit us in office to finalize the adoption process.

In the event your animal must be returned for any reason, we will accept the animal back in to our facility, but your fee is non-refundable.

By certifying and submitting this application you agree by our terms and conditions and understand that false or misleading information is grounds for denial and submission to the property authorities.
